DAY 4: Email notification will be sent that your Medical Opinion is complete and a final fee of $50 is due.

Day 5: ELITE Report- If a written report is requested,($200) ,it will be emailed to you. You have the option to schedule a video meeting with your ELITE Specialist. Pay the fee of $50 per 20 minute increments.
VIDEO MEETING
Appointment Day: Your ELITE physician will discuss your review and answer your questions.
The user can cancel the video meeting service after formal request and get a refund by sending an email to ELITE contact more than 24 hours prior to the appointment time.
Refund/Cancellation policy
Refunds can be issued anytime before submitting information for review by ELITE physician. No refunds can be issued after submitting data for review or retrieving the report. Therefore, all credit card transactions should be considered final and nonrefundable.
